Appointments are usually scheduled on a weekly basis. Every effort will be made to determine a consistent time for appointments. If you need to change an appointment, a 48 hour notification is required.
My fee is $150.00 for 50 minute session. I will complete all required paperwork (and/or electronic billing) and help facilitate reimbursement of 3rd party payers; however, I will expect direct payment from you and cannot guarantee reimbursement.
